Add comment buttons are confusing

Bug Description

There is a "document with plus sign" icon in the header, which opens the "add comment" popover dialog, and there is also an "Add comment" button which does the same thing. Why are there two?

In addition, the 'Add comment' button looks disabled, because it's grey. It's not clear it can be used.

This might be caused by the issue just present in the fork. Let's rebase the fork and fix the coloring if needed.

As for the 'Add comment' button at the bottom, I won't be doing anything about that, as it landed in a trunk while ago.
After "Ubuntu Community Testing" is rebased on mainline Checkbox, the issue will be gone.
