Bug Description
Using the Alternate CD to do an upgrade to 12.04 and not used the network there are missing files:

A quick find shows differently named files in the image:

Downloading the kubuntu precise i386 ISO and mounting it, I see:
$ find /mnt/ -name \*xserver- xorg-video- radeon_ 6.14.99\ * main/x/ xserver- xorg-video- ati/xserver- xorg-video- radeon_ 6.14.99~ git20111219. aacbd629- 0ubuntu2_ i386.deb
/mnt/pool/
$
This looks to me like you've mounted the CD with the wrong mount options for extended filenames, not like a bug in the CD image. If the filenames were wrong, it would also impact users' ability to install from it at all, and no one reported any such problems. What are the mount options you're using for this CD?
