Password reset is broken in multiple ways

There are three main issues with resetting a password:

1. There are no links to reset the password on the Home page/FAQ/login/Register pages (or anywhere else obvious).
2. Once someone does happen across the password reset page, it requires you to remember your username. It should just require an email address - it then should email you a link to reset your password (like the current system on launchpad does). ...or pretty much every other website with passwords does.
3. The password reset isn't working at all. Even passing hurdles 1, and 2 above the username and password that gets emailed doesn't work.
