Bug Description
If a XHTML-document is created with tbtohtml file split-level=chapter there should be an xhtml-file with the generated index for the document, but this index-file is missing. Index-generation works fine without the split-level option.
(P.S: tbtohtml split-level=chapter does not produce a correct makepage script: Only the master file (=toc) is copied to the public-page folder)
