Profile setup: move "your name" close to "username"

In the profile setup page, these are the questions:
your name:
your server's name:
pick a username:
(all about the user from now on)

I suggest to move "your server's name" to the first position, as all the other questions are about the user. In that way, they are grouped together. Otherwise, we go from my full name, to a server's name, and then back to me.
