PatchApplier should only add the patch at the end

Bug Description
Right now, it adds the patch right before starting to apply the patch. If the patch does stage commits, it's going to be persisted to the table, and if the patch fails after, it will appear as applied whereas it's not completed.
We should add the patch at the end, so that it's added on success.
