enhancement: add comments to authorized_keys stating the username

Bug Description

Many ssh keys have non-identifying comments, so when looking at an authorized_keys file generated by ssh-import-id it is difficult to tell which keys belong to which users. Adding a comment above each key identifying the username would sure be helpful.

  - support key revocation through a -r|--revoke option
  - only revokes keys that we label as adding (starting with this release)
