create tests for autotocs

Bug Description
We need a test suite for autotocs, and the tocrendering adapter.
It needs to test:
1) tocs in documents
2) AutoTocs
3) ensures all of the features of tocs work
and also verifies that index objects aren't rendered in the output.
(note, in 2.2 the behavior is planned to change a bit, with the toc container being displayed when it is a vhostroot or the silva root (i.e. the toc is a "sitemap"). OR, the toc container display is an optional checkbox.
