toc's should optionally display the toc's container

Bug Description

If the toc is a sitemap, it is important that the site root (usually the toc's container) is linked to in the toc.
Currently the toc's container is not displayed in the rendered toc.

There are two possibilities for implementing this feature:

1) automatically display the container of the toc if the container is a virtualhostroot (SilvaLayout MUST be installed) or a Silva Root.

2) expose this as an option in the UI

My vote would be to expose it as an option in the UI. Could be that a toc is used in a vhr, but it's not a site map, so having the container automagically display wouldn't be desired.

Would this mean that there's another level of indent?

The TOC code source already does this, via the "show headings" and "link headings" options. I'm adjusting the AutoTOC public view to do the same, will checkin this week.
