Action state can become stale while in queue

Bug Description

When an action is queued to be processed for events such as policy_attach it can become stale. If the state of the cluster is updated while that action is in queue it can cause the cluster to get bad/old data written to it in the database when the action is eventually processed.

    Add entity refresh to cluster action execute wrapper

    This patch adds a cluster entity refresh to the cluster action
    execute wrapper. This change makes sure the state of the action
    does not become stale while in queue.
