non-consecutive or negative raw jpg leaf num - malformed spread index

Bug Description

raw leaf numbers are becoming malformed in two, and perhaps related, ways:

1) a spread is skipped altogether - that is, the numbering skips, while the captures were successful.

from the apache log:
time 1315410878 shot annualreporttown1914hins spread 0 both
time 1315410892 shot annualreporttown1914hins spread 2 both
time 1315410899 shot annualreporttown1914hins spread 3 both

2) the spread index begins at -1
time 1315612061 shot annualreporttown1986jaff spread -1 both

resulting in numbering that looks like this:
