default picklist item can lead to a dup upload

Bug Description

When a scanner chooses an identifier from the picklist for scanning pages injto,
the first one may get used accidentally. That is, the first identifier is selected
by default, and if the scanner just presses the open button without checking
what's in the dropdown, the first name listed will be chosen.

This can lead to dup uploads because everybody in the same scan center
will see the same first selected item.

One thing we might do about this (per Hank), is to make the first string
listed in the drop down to be something like "Please select an identifier",
and have the code be on the lookout for that phrase: if the identifier
that gets selected and open'ed is "Please select an identifier" then we
could throw up an alert panel that asks the user to please choose one
of the (other) identifiers before scanning.

Slight clarification on the behavior observed by Andrea: in the case she described, an identifier may have been chosen, but the selection is lost upon refreshing the page. It sounds as though the menu reverts to the default state.
