Bug Description
Currently when you set a custom selection list field, its first value is chosen by default in the add forms. This could lead the user to accidentally set unwanted values.
I think the first option needs to be a promptMessage like '-- no value --', so if the user submits the form None is set for non-required fields or he will get an error for required ones.

Required demographics fields now display "select a value..." message, which is part of z3c.form. I'll set an appropriate translatable prompt message as part of my batch of i18n fixes.
