Skill levels should be generated automatically from Quiz data

Skills should be able to be derived automatically from quiz data by mapping functions on number of items on a given skill taken, number correct, and number correct on the most recent n items to a skill score.

For example, given a skills score system from 0 to 4, derive skills scores using the following rules:

0 - No items taken
1 - At least one item taken
2 - 10 or more items taken or 2 or more items correct
3 - 80% of the most recent 5 items correct
4 - Only a teacher can give a 4
