manual: document xref (incl. limitations)

I cannot find any information about SBCL's Xref support in the manual. In particular,
I'd like to find some information about its capabilities and its known limitations.

At work, I often face the problem that Xref information is poor, and I don't know
yet why. I'm not aware of any global optimization switches.

The whole of SB-INTROSPECT is missing from the manual.

Also, test-cases where XREF information is poor would be nice. There was a recentish bug that was fixed in 1.0.31.13 -- maybe that was what was affecting you? If not, it may be another bug...

Notice that I didn't have SB-INTROSPECT in mind but more an description
on what the compiler does for xref support.
