Missing slider on shift templates

The facility activation sliders disappear from the shift templates as user adds 10+ new shift templates.

Steps to reproduce bug:
1) Create a new scenario.
2) Follow the Scenario Creation Wizard to step 6.
3) On Shift Templates Page (step 6), click on "Add shift template" button to add 10+ more templates.
Notice the newly added templates are missing all the sliders on the right of the templates.

Nice catch.

I have looked at the issue this morning, however it appears that we are out
of sync. The javascript and the style sheets, most notably, are very
outdated. In order to fix/patch this (I would love to, as I have also taken
interest in doing this:
http://www.rhok.org/problems/shift-visualization-sahana-agasti-mayon-disaster-preparedness-and-response-software)
I would need the Mayon trunk to be updated. If this could be done by
today I could take a look at it tomorrow morning. Can you please push to
have that done?
