speed Up (enabled) check for available cases by applying questionnaire.enabled = 1 and samlpe_import.enabled =1 filters to querries in nocaseavailable.php

Dear Adam,
That's not, actually, a "bug" - it's more a vote to revoke commented issue in rev.443.
I liked this additional check. Just wanted to note that applying mentioned filters let minimize querry time consumption.
