Qpdfview v0.4.18
Arch Linux 5.8.10

Opening a pdf containing forms data does not display the forms content with the exception of radio buttons which are shown as a tick graphic. Forms fields ARE shown with a blue outline.
Left clicking on any completed form field displays all the fields in the document for editing.
Printing the document does not print the forms data, which I understand to be a CUPS problem.
The result of this is that when I received a pdf containing completed forms data, I was unaware that any data was present and thus returned the incomplete printed form.
Since printing a forms pdf does not work at present, either displaying the forms content, or at least a message to say that the pdf contains forms would be helpful.
