layout of qsend dialog is bad

Bug Description
None of the fields line up with each other and the dialog just looks really messy.
For example, the Address, Message, and Filename fields do not line up along the left edge. The Submit Branch and Public Branch also don't line up along their left edge.

In particular, the dialog is overly complex for normal usage. The Merge directive options should be collpased by default and only displayed if an 'Extended' view is requested.
