Bug Description
Example item: http://
I uploaded an .iso. Before derive was complete, I clicked "Edit Item" and added a .jpg.
A checkin task was then queued, which ran after the derive completed. This checkin deleted the derivatives:

A workaround would be to not launch the "edit item" thing until all pending catalog tasks had been finished.
