Bug Description

The OUTPUT section in pt-upgrade's manual seems broken. It lists a TODO section and then jumps straight into the option detail: http://www.percona.com/doc/percona-toolkit/2.0/pt-upgrade.html#output

Realized the OUTPUT section is marked as TODO when the manual is consulted with perldoc, so this may just be a bug with the template for the HTML docs.

I wrote a very basic OUTPUT section for pt-upgrade. Given that it's never had one until now, anything is better than nothing, but we really need to overhaul this tool, too. The HTML looks good locally now, so it should look good at percona.com too.
