Bug Description

Having run pt-query-digest 2.2 against a slow query log, I get the following (anonymized) output:

Notice 19-24 where it skips 21 and 23. They also do not appear later in the report. What gives?

No idea atm why this would happen, either some bug or some code I have forgotten about.

Ryan: has this ever happened before? I don't recall any similar bug reports.
