A patch to enable termal reliefs on SMD pads

Bug Description

I always wanted thermal reliefs on SMD pads. Maybe you find it useful. If so I will adapt it a little that it is more cleaner and incorporate it in the existing polygon functions (most part of what I added is copy/pasted). I don't think a lot of fundamental changes are required, I only added an extra type in const.h to be able to find the pads and added some extra arguments to the pad-polygon subtract function.

two bugs: the X-shaped thermal for round-end pads should have the spokes drawn from the centers of the ends, not the center of the pad. Second: a row of thermals on, say, a TSSOP (close-together long thin pads), then ctrl-K a couple times, PCB starts consuming memory.
Requests: a thermal type that looks like a pad-width trace extending the ends of the pad (no connection on sides) for TSSOP-style pads.

This patch still has some issues and needs a bit of rework, but I hope I will take care of it soon.
