Registration page 'Add New Affiliation' button link to home page

Bug Description

When you register the openstack foundation members,it need to choose the Affilation,but the 'Add New Affiliation' button is jump to the home page directly.

Hi - You are non-supported versions of FF, Chrome, and Windows. I'd be happy to add your affiliations for you if you'd like, but unless you can update your browsers, it's likely the javascript is throwing an error.
