I suspect some answers were removed but somehow still taken into account when filling the form.
In my case, the priority started at 4 until I clicked "clear all" as suggested by Tom.

This was actually a known .js bug that was patched a while back. If you visited the survey previously, I suspect that you had the .js file in your browser cache. We're also not able to replicate this one, but if you see it again, or can provide any other info (screenshot, etc..) that would be great.

OK, good news. We managed to reproduce it using a production db dump. There was a bug on survey prepopulation process that insert old values to businessdrivers fields and widget logic did not have in consideration that corner case. Basically, the responses were not shown on UI, but widget thought that already were answered.

The problem is fixed and we should deploy shortly.
