Bug Description

In order to submit code for review by community, it is mandatory to be Openstack Member. This membership is availed by submitting membership application in openstack.org/register page.

However, few of us affiliated to Hewlett Packard (HP) tried to submit our applications to send code for review.

After the buggon 'Submit My Application' is pressed the same page got loaded again without report if the submission succeeded or failed.

After 24 hours of time, we rechecked whether our profiles got enrolled into membership of Openstack Foundation here:
http://www.openstack.org/community/members/

and we could not see our names there.

Due to inability to enroll our names into openstack foundation as members, we are unable to send git review to community for code contributions.

Please request to take a look.

I cannot replicate the issue. Can you please add information about your browser? have you tried clearing the cache/cookies? A different browser? Do you get any error message? Can you submit a screenshot of the screen where the process stops?
