Incorrect/confusing "Same username" in "First timers in OpenStack Documentation Contributor Guide"

- [x] This doc is inaccurate in this way: The page has a diagram https://docs.openstack.org/doc-contrib-guide/_images/workflow-diagram.png. It reads "Same username" between openstack.org and launchpad.net. However, there is no "username" field at https://www.openstack.org/profile. Please clarify.

Do you think the diagram meant "Same username" between https://review.openstack.org and launchpad.net instead? If so, what is a user supposed to do, if
1. she already chose a username at review.openstack.org (and can't change it)
2. the same username at launchpad.net is already taken by someone else.

    [doc-contrib-guide] Update set up instruction

    The Contributor Guide has a good instruction now on account setup, link
    to it and remove outdated instructions.

    Remove now unsed graphic.
