Add a note indicating removal of PowerVM hypervisor driver

The maintainers of the PowerVM driver have proposed that it be removed
from Nova.

The User Committe rep Tim Bell has indicated that based on survey data that "this would not cause disruption for the deployments who reported their configurations."

PowerVM was mostly documented in developer docs in the nova repo. It also has entries in the Configuration Reference in the nova.conf options tables from our autodoc process.

Keep those tables in the stable/havana repo, but add a note to stable/havana with a clear message as to the driver's fate. We also have a mention of PowerVM in stable/grizzly where we could also place the same note.

    Note the removal of PowerVM driver

    PowerVM has been removed from nova. This adds a big warning
    to encourage people not to use it.
