Bug Description

Changes to the images like covers and pictures of authors should appear in recent changes and revision history pages. We need to be able to see when an image was changed, and by who. It might be nice to add a change summary field, so we can see why somebody has changed an image.

Though it could be improved. For example, a note of the source image URL (or a note that it was an upload from the user's local drive) would be ideal.
