link fulltext for books that have it online

Bug Description

There are a lot of books that have fulltext or pdf's online under conditions that we can't import them directly into the open library, but we can link to them. Examples:

  http://www.inference.phy.cam.ac.uk/itprnn/book.html
  http://www.cl.cam.ac.uk/~rja14/book.html
  http://www.monad.me.uk/stable/Proofs+Types.html

Maybe we could have a bibliographic field giving the url for this situation. The has_fulltext search facet would expand from available/not available to "in OL", "on the web", and "not available". I don't know if we should try to fulltext-index such books.

I agree. the online books project has a large amount of structured data
on this subject that I want to import for this.
