Fixed ip is not disassociated when instance is automatically deleted due to unresponsive host

Bug Description

When a compute host dies, the instances will automatically be deleted as well. There's a call to 'db.instance_destroy' in the compute api that does this. However, it does not appear to disassociate any fixed ips. This not only eats up one of your fixed ips, but it causes issues for Horizon when it tries to display associations of floating ips.
