Bug Description
When upgrading from 5.6.x to 5.7.12, the upgrade script does not properly initialize the tables required to have multi-source replication working, regardless if replication was configured in the 5.6-installation or not.
After upgrading, it's possible to create multiple replication channels, but they will all be displayed with the same name (the name from the first channel configured) in all ways, except in mysql.slave_
Both channels can be manipulated with giving the correct channel name in slave-commands.
After instance restart, all channels except the first one disappears.
