"Tap to shuffle music" breaks app (startup)

Bug Description

Today my music stopped to play unexpected, still don't know why. (Indicator prev-play-next controls also where disabled)
I closed the app. As far as I remember I deleted the queue and tapped "tap to shuffle music".
App then started playing for one or two seconds and than stuck. "Queue" tab entries are "invisible" (no text, but an entry is highlighted as if "currently playing"), not sure if title and artist are shown in "full view".
Closed app again and reopened and queue is empty. "tap to shuffle" still leads to the hang. Repeated some times, every time same issue.
When I select a song and play it, empty queue and then tap "tab to shuffle music" everything works fine again.

I haven't seen playback stop like that, but in some very rare circumstances I have seen the empty playlist entries before. This most likely is not music-app's fault but I'm keeping it listed here as a possible culprit just in case.

These crashes now happend to me multiple times, but today even multiple restart's don't help, the app still crashes.
I've attached by log, I can't tell how to reproduce, listend to music all day long and then unexpectly stopped.

When you get into this state and you're seeing the "Failed to query track..." log messages, try to see if you can play a video from the video scope (not via the web browser) at that time. Or another thing to try is previewing a song from the music scope. Do either of those work then?

Neither Music nor Video scope does work.

Also I've seen that in Log now (was also there earlier; it's spammed 100x or so)

Failed to query track for URI: org.freedesktop.DBus.Error.ServiceUnknown: The name core.ubuntu.media.Service was not provided by any .service files

afterwards keep spamming withFailed to query track for URI:

org.freedesktop.DBus.Error.UnknownMethod: Method "GetTracksUri" with signature "s" on interface "org.mpris.MediaPlayer2.TrackList" doesn't exist

Hello,

I think I found the source of my issue:

    [EE 2016-08-13 08:19:05.875356] [engine.cpp:140@from_gst_errorwarning] ** Encountered a GST_RESOURCE_ERROR_READ

When I `cat` the file, I also get an I/O error

    cat: 14. The Other Side.ogg: Input/output error

But when I `find /media/phablet/[sdcard]/Music -type f -execdir sh -c 'cat "{}" >/dev/null'`, all my files have an Input/Output error.

So I think it's more a problem w/ my SD card then w/ Music App or Media Hub.

M SD Card is vfat, so maybe it's a problem with the file system? syslog for a single `cat` says:
