Produce an annotated meeting log

At the moment, mootbot produces a summary that often contains too little information (e.g. if people forget to record decisions with [AGREED]) and a full IRC log that has no structure.

It would be nice if mootbot produced a hybrid log of the meeting:
 1. contains the full IRC log
 2. Creates headings for each [TOPIC] line, possibly with a table of contents at the top
 3. highlights [IDEA], [AGREED], [ACTION], etc in boldface or similar.

This would be much easier to scan than the unformatted IRC log.
