clean up and update skin documentation

Bug Description

http://mixxx.org/wiki/doku.php/creating_skins has lots of outdated information, examples, and screenshots. It would be helpful to remove the clutter of legacy information and refocus it on current best practices.

Absolutely, thanks for the reminder.
Users interested in modding/creating skins should not have to dig in the old dirt.

What i´d like to keep is the ``changed/new`` section, that give a good overview of thing that differ from previous versions.

Anything special in mind about the structure of the document?
