Bug Description
Here is a transcript of a chat session with Mixxx:
[13:47] <mickey> can anyone tell me why Mixxx will play a track twice back to back in a set playlist?
[13:56] <Pegasus_RPG> mickey: unless it's listed twice, that sounds like a bug
[13:56] <Pegasus_RPG> Is it playing every track twice?
[14:01] <mickey> not every track, but more often than not
[14:15] <Pegasus_RPG> Hmm that's a tricky one to debug... does the console output show anything?
[14:17] <mickey> it just loads the duplicate track into one of the decks. After it plays the second time, it moves on to the next file.
[14:18] <mickey> Additionally, I am trying to use Mixxx to live stream and it wants the LAME file. I have tried to use the ones listed through your website, but after downloading he zip file, it says it can't open the files to extract. I'm using 7-Zip to extract
[14:19] <mickey> Lastly, when it does move on to the next file to play, it cuts off the first second or two of the track

Additionally, the latest incident is that the Auto DJ just suddenly stops playing anything, and is a bit of a trick to get it started again. I am using Mixx to handle programming on an FM radio station when we're not setting live in the control room, and it is a little upsetting that when I am away from the station and the sound of our broadcast suddenly stops, or double plays a track.
Mixx is operating on a Windows 8.1. I'm not familiar with the processor or hardware configuration, but the computer is an HP Pavilion 23 "All in one".
Which version of Mixxx is effected?
It there something suspicious in the mixxx.log file?
