Allow sso in both directions when usersuniquebyusername is on

Bug Description

In https://bugs.launchpad.net/mahara/+bug/547564 we removed the option to set up sso in both directions between Moodle and Mahara, because it doesn't work, it just creates duplicate accounts.

However if the undocumented usersuniquebyusername option is enabled, then sso in both directions can work and we should add a 3rd option to the drop-down.

We are focussing more on LTI than MNet and thus the bi-directionality needs to be rethought. As far as I know, you can end up in the same account via LTI as long as the email address is the same.
