Sign off block says "signed off" by default

Bug Description

Steps to reproduce:

Add a sign-off block to a page
View page in non-edit mode

Expected result:
 - Red dot saying "Sign off" to indicate that clicking the dot will sign off the page

Actual result:
 - Red dot saying "signed off", suggesting the page has been signed off, when it actually hasn't

To fix:
Red dot says "Sign off"
When clicked, this changes to the green tick and says "Signed off"

Then the same would need to apply to 'Verified' for consistency. Though in reality, what the current situation is saying is:

Signed off: No (red) / Yes (green tick)
Verified: No (red) / Yes (green tick)

Thus, the language is correct. However, I do see that this may not be clear and especially with the colour choice and both dots looking the same and the red one not having a second encoding like the green tick. I suggest we use the red dot with the x in it for not signed off pages similarly to the icon used in SmartEvidence to indicated that the page is not yet signed off or verified.

I think it would be good to use the words 'not signed off' for clarity. It may help to avoid those issues we had in the plugin administration with 'show/hide' and people struggling to understand the status of those buttons.

Perhaps we could have this:

Not signed off: red
Signed off: green tick

Not verified: red dot
Verified: green tick
