The 'accessfromdate' column is missing from the Completion and review report

This column was there in the initial commit https://reviews.mahara.org/#/c/11667/ but the code for the SQL part was missing - but then the https://reviews.mahara.org/#/c/11676/ patch removed the parts for the column rather than fix the SQL mistake

So this bug report is to put back the existing code and fix the SQL mistake

Note for testing: 'Log events' needs to be set to 'All' in 'Admin menu -> Configure site -> Site options -> Logging settings', but Elasticsearch is not needed.
