Bug Description
I've noticed a standard Mahara installation winds up with a different theme in 15.10, depending on whether it was upgraded to 15.10, or cleanly installed at 15.10. A 15.10 install from scratch gets the "raw" theme, while a 15.10 site upgraded from 15.04 or earlier gets the "default" theme.
I think this might just be a leftover from earlier on in the Bootstrap development process when the "default" theme wasn't bootstrap yet. We should probably fix it, though, so that the "standard" Mahara theme is the same one, whether you've upgraded or installed. So that either means we give new 15.10 installs the "default" theme; or on upgrade we change sites using the old "default" to instead use "raw".
(I know there's a kind of pedantic argument to be made that, if you're on default in 15.04 you should be on default in 15.10. But I don't think that's true, because the 15.10 "default" theme is completely rewritten and bears little resemblance to the "default" theme of 15.04. If a user is on 15.04's "default" all we can really say about their preference is that they wanted the theme that is there by default.)

To test:
1. Install Mahara 15.04
2. Upgrade it to Mahara 15.10
3. Take a screenshot of the site
4. Delete that site, and do a clean install of Mahara 15.10
5. Compare that site to the screenshot of the upgraded site
Expected result: Both sites are using the same theme
Actual result: The upgraded site is using the 15.10 "default" theme (with chrome highlights, and three big green circles on the dashboard); while the clean install is using the 15.10 "raw" theme (primarily white, and little green circles in gray boxes on the dashboard)
