Change "Image" block to have empty title by default

Bug Description
This is a suggestion that came up at the Mahara Hui 2015 in Auckland.
Currently when you create a new image block, its instance title defaults to "Image".
This is hardly ever the final image title that you want the block to have. Most of the time, in fact, to make a good-looking Portfolio page, you don't want the image to have a title at all.
So I think it would be best to change the Image block to have an empty title by default. It may also be true that other blocktypes (like Text) may also benefit from having a default empty title.

I thought this one would be bite-sized, but it turns out we currently lack any API for changing the default title of a block instance. See bug 1451329.
