"Shared by me" table should be simplified

Bug Description

At the moment, the list of pages in "Shared by me" is in an overly complex table, which is bad for responsive design. To simplify this the first two columns of the table should be merged, with the access list shown under the page title.

Without a screen shot I'm guessing the Shared by me page looked like: http://manual.mahara.org/en/1.9/portfolio/share.html

It now looks like http://manual.mahara.org/en/17.04/portfolio/share.html and is more responsive
