asks the user to input data that the form already knows:
Please ensure that all feedback contains details of:
* medium (paper or screen)
* application (including version number)
* for print please include make of printer and driver including version
In the common case, the medium (screen), application (this $browser/Firefox $version/x.y.z), and it (not being print) is already known.
There appear to be two use-cases that are being conflated into a single form:
a. Browsing glyphs in the browser form and immediately flagging up an issue
b. Noticing an issue elsewhere (eg. Print-out, Openoffice)
These two have separate workflows; idea the issue is in the automatic screenshot in which case the information is known and the user doesn't need to be asked to re-enter it (they probably don't know anyway). Or, the issue is only seen in another application or print out, in which case the user needs to attach a screenshot/camera photograph (the most import piece of information in diagnosis) and preferably also state where how.
