Linux Mint 11, 12
When Firefox is set to not show the menu bar, a bookmarks item appears either in the bookmarks toolbar or the navigation toolbar, depending on if the bookmarks toolbar is open. This appears to have the default Firefox or Gnome icon, so it sticks out like a sore thumb.
