Hi, thank you for reporting this issue!

In order to address this issue we need to know a few things first.
Could you please provide a copy of /var/log/syslog so we can
see if anything else is happening first? In the mean time I'll
take a look at Ubiquity's code to see if anything is happening

I'm having the same problem with the Mint 12 installer:

Description: Using Mint 12 x86_64 DVD, the installer permits entering a username containing an underscore (eg test_underscore), and continues to the next menu (choosing a picture). On selecting a picture and clicking "Continue", a dialog box appears stating "The username you entered is invalid. Note that usernames must start with a lower-case letter, which can be followed by any combination of numbers and more lower-case letters." There is one button, "OK". On hitting this button, the dialog closes and immediately returns, making it impossible to click anywhere else in the installer. This loop continues until killed with xkill or similar. If you wait, after a while ubi-partman fails with error 10 in the background.

Always repeatable on my system.
Note that it does not have a problem with dashes in usernames, only underscores.

Workaround: Don't use underscores in usernames.

/var/log/syslog attached, on the off chance it helps.
