remove the use of ACPI_BLOB_HEADER_SIZE from all code

Bug Description

Since early_ioremap() and __arm_ioremap() are now being used to pass in ACPI tables, there is no longer a requirement to have a header on the beginning of the ACPI blob being passed into the kernel. This bug is to make sure we remember to remove all use of ACPI_BLOB_HEADER_SIZE from ACPI and GRUB.

NB: the GRUB2 code currently checks to see if an "ACPIxxxx" header is present and does NOT load the tables unless it is.

This is apart from mab and kernel changes needed.
