Bug Description
The FormOverlay passes an argument to its submit callback with the data from the form. Unfortunately, since the API wants choices in their display text format, a hack needs to be added so that data is passed the proper value.

Could you expand on this? I'm fairly sure thing are working without it, so I'm inclined to retriage this to low.
