Chameleon files with an XML header and no encoding causes an encoding of "None"

Bug Description

If you are writing an XML file with an XML header and do not incude an encoding, an encoding of "None" is added automatically. This can cause surprising errors. lib/lp/testopenid/templates/application-xrds.pt lib/lp/testopenid/templates/persistentidentity-xrds.pt are affected by this.

As a workaround, simply add an encoding to your XML file. This is strongly encouraged as best practice in any case.
