Add unsubscribe link in bug mail for structural and implicit subscriptions

Bug 48863 notes the need for bug mail link to link to an unsubscribe page. This has been done for direct subscribers but some work is needed to make this usable for implicit or structural subscriptions.

Ideally, the unsubscribe link will be included as part of the rational, i.e. "You are a direct subscriber of FOOBAR, click here to unsubscribe."

Well, except this is an email, so there's no "click here" (and even if it was the web, you'd lose brownie points on accessibility in using that phase). Guess a URL will have to be included.

Yeah, that wasn't a very good sketch of the idea. I'm sure I've been
guilty of a "click here" in my web lifetime, but I didn't mean that
actual phrase when I wrote that. I was just trying to capture the
idea quickly while Tom and I were talking about it.

To be more accurate, I think this should take the form of the current
link for direct subscribers. For example,

"You received this bug notification because you are subscribed to foo.
 To unsubscribe, go to: [URL]."

Maybe each rationale requires including the URL in a different way,
but maybe not. Anyway, we should be mindful of the phrasing as we
develop this and it goes through review.
