Bug Description

Administer PO template form needs a unique (productseries, potemplate) validator

Steps to reproduce (using sample data):
1. http://localhost:8086/products/evolution/trunk/+pots/evolution-2.2-test/+admin
2. Fill in Template Name: evolution-2.2
3. Fill in some header information (the content doesn't really matter, if you leave it blank other validator will be triggered.)
4. Make sure Product Branch or Series is: evolution/trunk
5. Click on change
6. Crash like this: OOPS-131C340

That unique constraint may have been lost during our schema refactoring. We should reinstate it as well.
